I'll answer for synagogue:
It's about a 25 minute drive without traffic.
We probably go about once a month. It's tough to come all the way home (Tukwila) and then drive back north (the temple is in Wedgwood), so we really only go if services start early.
Distance definitely affects how often we attend, but we unfortunately don't have a lot of options. The temple in Tukwila is Messianic (aka. Jews for Jesus) and the next closest temple is basically a Jewish mega-church that makes me profoundly uncomfortable.
It's about 15-20 minutes. I help in the preschool program once a month but we haven't gone very consistently otherwise in quite a while. It doesn't have to do with distance though, it's that we need to find a new one and aren't happy with the direction our current one is heading.

We don't go to church with any regularity. When we did, it was about a 10 minute drive, then we moved and it became a 25 minute drive. That pretty much put an end to our attendance, and we haven't found a new one we like.
Google maps says it's 13 minutes in no traffic, but when I go with babyniq I leave 30-40 minutes before I need to be there. I go every week. I'm in the church choir; if it were more than half an hour I would probably look for an Episcopal church closer to home, even if it's not the cathedral.

Jews for Jesus are effectively Christians who consider themselves Jewish by birth. I find the movement to be profoundly confusing, and also in direct conflict with many of the main tenets of Judaism.

We haven't attended Mass since we moved. We're terrible Catholics. But we need to get B baptized soon, so we'll start attending a parish that's about 10 minutes away. If we don't like it there, we can go to another parish that's about 20 minutes away in the other direction.
