If you think they didn't run it through insurance or didn't run it right, I would call the insurance company and they can tell you what they still need from the doctor/hospital, etc.

Yes, they have to give it to you. Go over it with a fine tooth comb.
Also, not sure of your DHs job has this as a benefit, but ask him to look at his, and see if they have a contract with a healthcare advocate company. A.on is who my DHs company uses. They can help with billing issues, medications, treatments, etc.
I hate insurance companies with the fire of 1000 suns. But yes, they should absolutely be able to provide you with these details, and if they billed incorrectly (outpatient as inpatient) they should resubmit the bill correctly before making you pay.
