We have had only two experiences with ear infections: DD at 18 months, which healed on its own within a couple of days and DD's friend who landed in the hospital and almost died. So, I need help knowing what's normal.
We went to the doctor last Friday after DS was up half the night with a fever and screaming. Double ear infection. They said they saw lots of puss. Looking back on the week, I think he was probably dealing with it all last week and we just attributed it to different things. He's now been on amoxicillin for 5 days, and today they said they were still red and bulging. They didn't seem too concerned. No more fever, but he is still cranky and sad and very tired. Not a huge appetite. So those with more experience here, does that all seem pretty much on par? The doctor we saw today (not our regular pedi) didn't seem surprised by anything, but I want to hear it from some mamas.
DS diagnosed with "bad" double ear infection in Thursday.
We left to go to the mountains. He was super fussy and not eating so I took him to urgent care (actually it was the small city's ER) on vacation on Tuesday (so 5 days) and he still had red/bulging ears.
ER dr wasn't worried and a few days later he was back to his normal self
So it sounds almost exactly like your scenario.
DDs took long to go away too and she had two courses of antibiotics close to each other.
Just keep an eye and take him for a recheck in a few days!

Thanks, guys. Sounds like he's totally normal then. Hopefully he turns a corner and perks up soon. At least he's taking his amoxicillin like a champ (knock on wood).
We have had many EIs over the years, myself included as a kid. Short eustacian tubes can be genetic so we've had plenty. Always go back for a re check within 2 weeks, it's not uncommon for a second round of atbx to be Rx. Ibuprofen can help with inflammation and getting better sleep. Waking in the night was always an indicator of EI for my kids. Hope he's feeling better.
