No, I've never done a wardrobe change between a wedding and reception and i dont think I've ever seen any friends o it either. But maybe we are not doing it right!
No, the only times I see people doing that is when they wear a suit or something proper like that for church and then change to a cocktail dress. I am too lazy and just throw something on my shoulders at church.
I have when there was a decent gap. Not because of the gap so much as the difference between tones between the wedding and reception. The wedding was a late morning Catholic mass, and the reception was a really upscale evening affair.
I don't see anything wrong with it really, even if you could wear the same dress. I mean, I'm always game for a reason to get a pretty new dress.

The girls in my family will do it sometimes - "Sunday best" for the church and cocktail dress for the reception. Sometimes the boys won't wear a jacket to the church but then put it on for the reception.
The only time I did a wardrobe change was for a Halloween reception. The ceremony was in a church so formal dress code. We changed into a samurai and a geisha lol!

Always. Mexican and/or Catholic weddings always have a gap and I normally wear something church appropriate for the ceremony and slut it up a bit for the reception.
