I wouldn't think so...we did mixed bottles for a long while. We would mix and shake up the formula first, then combine it in a bottle with the breast milk, but we never shook up the breast milk.
Question about this, I'm not really keeping up with myP's demand. How do you mix bottles? Like, 3oz breastmilk, 2oz water + formula for that much water?
When my kids were in the NICU we fortified my milk with formula. So I assume fine. I was only adding 1/2 a tsp at the time since they were getting smaller bottles. I don't think you are hurting your milk, only downside would be you probably can't save it if he doesn't finish but my kids rarely don't finish a bottle.

Mix the formula separate and add to the BM. I would make whatever formula I needed for the day in a bigger container, mix, then divide into smaller bottles.
Brie-It doesn't matter! Give him whatever he'll eat with the least amount of waste. I preferred to give DS the same bottles all day long (which were all mixed) so taste-wise it was all the same and I didn't have to deal with him potentially turning his nose up at one that was all BM or all formula.
