Im really annoyed . I just drove into work so that I could submit my Receipts for my FSA money (I cant scan at home), only robotics that the recipes all are wrong AGAIN. My FSA requires the dates of service. Is that how yours is? To me it seems like that would be the norm . But I keep having to ask the director to include that on my, so mine are "special." So all mine have the dates of the last time I requested that she include the dates and now I've wSted this stupid trip. And the director is out of office the rest of the week. I should have looked at them more carefully, but I'm still annoyed. Including dates on a receipt just seems normal.

that's annoying though if you are asking for the receipts and there are no dates on them. I can't believe your daycare doesnt have any other parents who require the dates. I would think it would be standard for FSAs.[/quote]
This is totally where I'm a at. How can I be the only one that needs this?! To make it even better, the first ones didn't even indicate I had paid. They were simply invoices, but she was calling them receipts. Now she adds "paid" to mine. again.... How can I be the only one?!
Our daycare gives out invoices (which do not indicate we've paid!) and then a handwritten receipt book thingie that doesn't include the dates of service. When I need FSA paperwork they write up a letter that says "The Smocks paid $X for service from date-date for Baby Smock." So far that works, but it would be a pain in the ass if I wanted to get reimbursed more than once a year!
I write the dates of service on my DCFSA form. Our DCP signs the form, no receipts.
Same. I fill out a recurring reimbursement form once at the beginning of every year (dates 1/1/XX - 12/31/XX) DCP signs & then we're reimbursed every pay period. Easy peasy.
Yes, my FSA administrator requires dates. You're only supposed to use the FSA funds for expenses incurred during the year you get the funds, i.e. no using 2015 funds to get reimbursed for expenses from 2014.
Your DCP is being unreasonable. Tell her to get a new receipt that has a line "for care from _______ to ______".

This... I have the option of using the receipts (which do have the line Niq) suggested above, or I can fill out a separate form, have daycare sign off and call it good. I have done it both ways without questions, the little receipts daycare uses are often difficult to scan in, so I just opt to have her sign the form every few months and claim away!

I only submit a few times a year, and each time it's like whoever I got at the business office is all, "OMG! THIS IS ALL BRAND NEW TO ME!" So, what I have taken to doing is to attach the last letter to my email, and just say, "Use this format and send me and updated one that covers the dates from x to x." The letter template has my name, the kid's name, the Tax ID Number of the place, the dates of each week, and the amount charged each week.

This is what so annoying. She has updated the template (for us only, apparently) and there is a line for dates of service. But I think what's happening is because she doesn't enter it for anybody else, she forgets to go in and change it when she gets to ours, so it all defaults to the last time she entered the dates, which was 2014 ^o)

As I mentioned today, I need to get a receipt for all my payments that include:
1. dependent name 2. service dates (begin and end)
provider name provider contact information description of service expense amount
The top two items are the two that are not listed on the receipts I have received. I will need this information on all future receipts as well.
