Yes, as part of physical therapy. The two times I was pregnant and needed PT (once because pregnancy aggravated a pre-existing back injury and the other time because I had diastisis recti), massage was part of the therapy and insurance covered it at 80%.
I also went to see a rolfer about the back injury back in the day and insurance covered a large part of that after I met my deductible. You might look into rolfing for your chronic issues (google it).
Yes. I think we each have a limit of like $800/yr. or so, but we can go anywhere, anytime, as long as the massage is done by a RMT. No other restrictions, no referral, etc. required. But neither of us have ever gone; crazy, I know. I need to get on that.
Yep! $20 copay until I met my deductible, then it was completely free, up to 40/plan year. And I took full advantage while pregnant. I went through my chiro.
* I have a HDHP now so I'd pay OOP for a while. But they are considered a medical expense if I get one through a chiropractor or PT office so they'd count against the deductible. 12/year are covered if there's a medical reason for it - chronic shoulder pain would be covered under my plan since that is a diagnosis.
