I had an office with a locking door so nobody probably.
I did take cat naps at work a couple times during the end of my pregnancy with K. My boss had a really comfortable couch in his office and told me I could rest if I wanted to over lunch.
I would be fired if I had a class of students. If it was my prep, then I don't think anything would happen. There was one teacher who would always lay down on the couch in our shared prep room to nap and it was awkward for the rest of us.
Door closed? Nothing. I would wake up when the phone rang or more likely, when I got an IM.
Door open? A CW would probably wake me. I think they would joke about it but it would frowned on/talked about. My boss is in a different office, but I imagine word would get back to her.

One of my bosses would see/ wake me. I doubt I would get into any trouble, I've been here 14+ years without it happening so I think they'd probably be concerned if I was ok

My first boss took a nap every day, about 2:00, just for about 20 minutes, same with my grandfather. Both owned their own companies. My old boss collected studies on how short naps increase productivity, there were a lot of them. The evidence that short naps help improve productivity is pretty clear (Obviously, some jobs its impossible, but a lot of jobs it is). There are even some companies that have nap rooms.
When I was pregnant, I slept during my lunch a lot of days. I had a couch in my office and sleep was more important than eating, so I would sleep for 30 minutes. I set an alarm and the bell for the end of lunch/beginning of passing period would ring, so I always woke up.

I would sleep in my car at lunch, almost every day, when I was pregnant. I just could not make it through! I would move my car away from the building into a shady spot, set the alarm on my phone and put the seat back
Oh also, it is very normal to take naps at work in Asia. There is a folding cot for each employee in our China office. They only nap right after lunch though.

If it happened during a class, my students would make a video and send it to all of their friends, and then I would get reprimanded or worse. During prep, nothing, as that time is mine to use as I see fit.
At my old job my coworker or manager would walk in on me, they would probably take a picture then laugh which would wake me up. Then they would go tell me to take a little nap on the couch in the break room- my old job was amazing!
