I'm glad you've got a meeting. Hopefully they'll take care of it for you. She has crossed several lines, and I'm sorry you have to deal with it.
If you choose to respond to her personally, I'd do it in an email. First (& most importantly) it's easier for you to figure out exactly what you want to say and you don't have to listen to her go on and on about how she just caaaaaaaaares about yoooooou. Also, then you have it in writing that you asked her to stop.
It was a catch up meeting to let me know that my work has changed and she wanted to check in on me to see how I was doing, which is why she got a room instead of having it at her desk. I said I was doing ok until....and we talked about it.

Just wanted to friendly punch you in the arm for 'Kudos to you; glad it's handled; I hate your bitchface CW' lemons. Deep breath, the day's almost over, I promise.

Oh my gosh, I am so sorry you are dealing with her. What an intrusive, inappropriate person. I hope your supervisor or HR intervenes swiftly and firmly on your behalf. If you don't know how to get in touch with HR, certainly your FMLA person will.
ETA: now I see someone already has. Good. I hope they are very firm and let her know exactly how awful she is being.

I'm sure you have learned this already, but it is perfectly ok to tell people exactly how you want them to treat you. I was very clear that I would start crying the second that anyone tried to comfort me so for everyone's sake, treat me the same. Or even be slightly indifferent to me so that I can do what I need to do to get through the day.
I told my best friend that all I needed to hear was, "This fucking sucks. I am so sorry."
I told my physiotherapist to be mean to me so he threw a tennis ball at my head every time he saw me. It was a good way for him to acknowledge what had happened without having to say anything. (My head was not injured, I am really good at catching things).
I won't have to see this CW again until next Tues. I'm taking tomorrow off...3 days of work in a row has been more than enough this week. And Friday I work from home.
Thanks again everyone! I was the one that made sure everyone treated H right but yet didn't have the strength today to shut her up. She seriously wouldn't shut up even with tears welling in my eyes.
