Ds' preschool teacher did. It was maybe 15-20 min, she had some paperwork to go over etc. it was mostly an opportunity for Ds to meet her again before school started in his own environment.
No, our daycare does not do home visits. I mean, they are super-friendly and involved and all of the awesome things you want a DCP to be. Sometimes if there are concerns about a behavior they will ask about what we do at home (what we feed him, how we deal with biting, etc), but they have never brought up the possibility of a home visit.
My dd1's preschool teacher does, but it's a small co-op. She probably spends 30 minutes talking with us-dd1 shows her toys, talked about what she did over the summer, and anything else interesting. Dd1 loves having her teacher come visit.
My DS's ECE class did this. It was through the public school and his teacher came to our house once a month to catch up on progress, go over questions, discuss any concerns, changes in the IEP, etc. The visits were about an hour and were really helpful. It kept the line of communication wide open.

No, but our second choice preschool did this. It was one of my favorite things about that school. I thought it would be really good for DD1 to get a chance to meet her teacher at home and get comfortable with her before school started.
At least for that school, it was just the teacher coming over for a short (maybe 15 minute) visit to get a chance to know your child and for the kid to get to know her. I assume she comes in and sits down with you and your kid while the kid kind of runs around and plays.
DD1's preschool teacher came to our house after DD2 was born (not a part of a school thing, just because she's awesome), and DD1 basically held her hand and ran all over the house showing her every toy she owned.
