103.5 for three days? I would have called yesterday. However, my kiddo is in daycare so he would be kept out and H or I would have to miss work for it. Going to the doctor makes sense when your kiddo has to be kept out of daycare...
DD tends to spike high fevers. I don't call the pedi unless she has a specific symptom besides the fever- a rash, sore throat, something like that. Otherwise, I call if it's been more than 3 days or so. It also depends on if she's lethargic or responding well to Tylenol/Motrin.

I make an appointment pretty quickly for a high fever. Especially with no other symptoms. Partly because she has a kidney problem that makes her more susceptible to UTIs but even without that I would take her in by day 2 of a fever.
Our rule is to take her in on the 3rd day. DD is prone to ear infections, so it's often that if it lasts more than a day. Sometimes we take her in on the 2nd day with any fever if we see her being clumsy, since she falls down more when she has an ear infection.
I probably go way too often, but because she can't go to daycare without being cleared by a doctor or being 24 hours without certain symptoms, I like to see the pediatrician early. If it's an EI or something else not contagious, I want to know ASAP. If I could be out of work with her with less anxiety, I would have different habits.
ETA Just realized your child is older. I'm useless!
