I had three - the initial appt where they evaluated and did the root canal, the second part and the temporary crown and then the third was the permanent crown.
I guess 4 technically. One was a normal dentist appointment and we found my back tooth was cracked. Another was to the endodontist for the root canal, one back to the normal dentist for the temporary crown and the last one for the permanent crown.

It actually took me TEN appointments to get a crown (no root canal) because there were issues with the mold not working, the color being wrong, etc. It was a total PITA, but 13 years later, my crown is strong as ever.
My dentist can mill the crown on-site, so no temporary crowns. Usually one appointment to diagnose, then one more to do it. I think most of my crowns have had pre-existing root canals, though (with previous monster fillings on top).
First one at dentist for check-up to confirm root canal needed. 2nd one at endodontist for root canal, temporary crown. 3rd one back at dentist to get mold made for permanent crown. 4th one back at dentist to have permanent crown installed.
I go to a small dentist, if I went to a big place they might be able to make the crown themselves but he does the mold and sends it out.
