I can recall faghag being used, in a derisive sense, by my dad's godson (who is now 63) who is gay, back in the mid 1970's. Perhaps as society has become more accepting of such things this is no longer the case.

My experience says that the LBGTQ community uses a lot of words within itself (for example, dyke) that aren't ok if people outside of the community use it. Sort of a 'take back the word and own it' stance that's supposed to be empowering. (This is largely based on my undergrad experience at the cradle of feminism, being bi myself, and my gay moms.)
I still don't like the C word, though, and I can't speak to fag hag specifically. I personally don't use it.
