We're just looking at AIs, as mindless relaxation is what we need right now. And just adults-only since the goal is to get *away* from kids for the week.
We've been to El Dorado Royale and liked it, and Excellence Playa Mujeres and LOVED it.
If you want to just relax and do nothing but swim, read, and play a little beach volleyball: Bahia Principe Cayo Leventado in the DR. It's on its own island, secluded and relaxing, great spa, good food, and the ATV day drip is worth it. We went for our honeymoon in March 2011. Lovely.
Sun Palace (same company)is also amazing, it's on the quiet end of Cancun, couples only, and has less than 200 rooms. I also just saw an article on MSN that listed it as having some of the best food for an AI.

Punta Cana, Dominican Republic: Iberostar Grand Hotel Bavaro
Negril, Jamaica: I will go with the RIU Tropical Bay Palace or the RIU Negril. These are located on Bloody Bay Beach, around the bend from 7 mile Beach. I think this stretch of beach and water is much better than 7MB. The water is extremely calm and clear. The only thing is that these are not Adults Only...but I will say that I hardly saw kids running around while there.
Riviera Maya, Mexico: I've never stayed at either but family members just got married at the Valentin Imperial Maya and raved about it.
Excellence Punta Cana. We were there last week and it was great, totally adults only.
We've also been to Sun Palace. It was nice but smaller and more hotel-like, less Caribbean-resort like. I didn't like the scenery as much there, and the service wasn't as good/friendly as EPC.
We also did Couples Swept Away for our HM in 2007. It was wonderful, but more $$ than the above choices.
