I will sometimes start the pointy bit of a slice with a knife and fork if it's a particularly large piece of pizza and not crispy (and then move to hands), but most of the time I just use my hands only.
It depends on the kind of pizza. If it's the super thick cheesy kind they have in my hometown, it's impossible to eat with hands. Most typical pizzas, hands.

I have a cross bite, and I physically cannot bite through melted cheese on pizza when it's still hot. If I try to take a bite at this point, all of the cheese and toppings sliding off the pizza, either onto my face or into my lap. It's much preferable to eat the pizza with a knife and fork so this doesn't happen.
Usually a fork, no need for a knife.. When I had braces I didn't like biting into the cheese, and just got used to using a fork. I will use my hands if I'm at someone else's house because they don't always offer forks and I don't care enough to ask for one, lol
