If a job posting asks you to include salary and reference requirements, where should you include it? In the cover letter? Somewhere on the resume? In a separate document?

I put the salary in the cover letter - always put a range and say it's negotiable dependent on total compensation package.
Are they asking you to provide references in your application? If so, I'd only give ones you're comfortable with them calling now just in case (i.e. not your boss) - mine are at the bottom of my resume but you could also include them in a separate document. If your resume has a "header" with your name and any design element, repeat it on the reference page to make it look cohesive (and also allow them to know who the references are for if the resume & reference page get separated).

I agree with all of this. Also, we are only interested in work related references and not personal references so be sure to include 3 people that can speak to your work habits. If you wish to include personal reference, add it as an extra one.
Ditto vanilla's advice. Unless there's a place to enter that information in an online application, I would put your references on a separate document with the same font/header/format as your resume.

I thought it was weird for them to ask for references in an application package, too... but, DH doesn't care right now, because it's general knowledge in his office that his job will be eliminated soon...
