So say you take your kid to their annual exam once a year near their birthday. Do you take them a different time of the year solely to get a flu shot? or do you wait to the annual visit and just do it then?

hmm... my pedi mentioned the importance of DD getting them annually but suggested waiting until her visit around her birthday. That seems like it would be too late.
Our doctors' office sets up a few clinics just for flu shots. They do it on a couple of Saturdays when they don't have office hours. You make an appointment just to go in for the shot.
I was going to get it at her annual visit (Nov) but as soon as I'm well later this week, she is getting it.

Well, not to beat a dead horse, but your choices affect the public. I am pretty sure I heartcosmos is the public.
With respect to my child, they have a clinic on consecutive Saturdays at my doctors office for the shot. Next year, we will go. This year, her birthday is in line with the shot.

If your child's birthday happens to be right during the beginning of the flu shot availability, then yes, I'd do it at their annual appt. But my kids' birthdays are at the beginning of August and the middle of Dec, so we either make an appt to get their flu shots at the clinic, or go to the minute clinic for flu shots.
My pedi has a clinic on three Saturdays for flu shots. If your child has their annual appt from the time they receive the shots (usually mid-August) until the beginning of Nov, they ask you to just do it at your regular appt and save the clinic slots for others.

This...except clinic weekends = no appointment needed.
Last year he got the first shot at a well baby visit and the second we just popped into the office and got (dr. said she'd fit us in whenever, and she did).
This year I think we can both just hit the public free clinic days they have around town. There's one a few blocks away. Mine, his, DH's all at once.
