I got a letter from a "subrogation service" asking for information about my recent ER visit injury (you know, the treadmill one) and if my injury was a result of an accident someone else caused. It sounds like they want to know so they can recover the costs of the medical care and pay back my insurance. Is this mandatory?
I don't know if it is mandatory mandatory, but I always do it. I got the same letter about my finger injury -- I think they just wanted to make sure it was my own stupidity and not a worker's comp thing.
(ETA: I've had to send a bunch of these out relating to my back/neck issues, and I've never heard back from anyone about it.)
They are just trying to determine who is responsible for the claim. As someone who works in the field, please fill it out asap--the longer you wait, the longer the claim takes to get paid.
I bet in your policy it is written that you have an obligation to assist in subrogation. I would fill it out so they stop sending them. They also may not pay your medical bills until they get that paper back.
We had to fill one out when my DH tripped over the dog while holding DS and we took him to the ER to get checked out. I think they are pretty standard.
If you don't fill it out, the notices will keep coming for months/years, lol.
This. DH sprained his ankle and I ignored the first notice. Then they kept pestering me. I finally called and it took all of 2 minutes to explain what happened.
I had to fill one out when I slipped going down the stairs and injured my elbow. I figured it was easier to just fill it out and let them know they weren't going to be able to get money out of anyone else than for them to keep sending me stuff.
Mine was sort of funny. They were asking about the foot I originally had x-rays for but I broke the other one later so I only answered for the first foot. It was a spur aggravated by running. I think I wrote something along the lines of, I hurt it by just being me.
