At the risk of sounding like an idiot...I was told by my OB that I am not immune to Rubella and that I will be vaccinated in the hospital after I give birth. My mom swears I was vaccinated...so what gives? Do vaccines wear off?
Some do, some don't. You may also have been vaccinated and for some reason *in you* it didn't take as well. I'm assuming if the dr said you aren't immune, they did bloodwork and didn't find a significant titer.
You can probably check the AAP or CDC websites to see how long they're supposed to last.
Ditto pps, they can tell with a blood test. I found out when I went to grad school that I wasn't immune to measles. I had to get the MMR shot then, after walking around for many years thinking I was protected.

After a 1990 outbreak, a second vaccination was added to the recommended MMR schedule. So if you were born before that and had only one, you might need the second. When I started college, my schedule was considered up to date. I transferred two years later, and it was during that time they changed the recommendation so to enroll at my new school I was required to have a booster.
That, plus sometimes they do wear off or don't provide maximum immunity, both of which would be reflected by blood work.
