Yes, in my family, any gift, no matter what size, gets a hand-written thank you. It's the way I was brought up, DH has grown accustomed to it, and how we will raise DS.
Depends who sent it. If it's my BSC nana then yeah, I'll write a thank you, put a recent pic of the dog/us in there and send it.
Other family, doubtful although I know my mom will LOVE getting thank yous from the baby. She loves when she gets thank yous from our best friends son b/c she spoils him too.
A phone call should suffice. In our families, people appreciate a thank you phone call (and therefore a chance to chat and catch up) more than a note. I think it's okay to do whatever you are comfortable with.

Depends on the giver. DD has been getting the occasional card/gift from friends of my DH's grandmother or mom who I don't know (and who DH barely knows if at all). If one of those people sent $5, yes, I'd send a thank you note.
If it was one of our parents, grandparents, or a friend, I'd let them know it arrived and say thanks via email or phone.
