I don't know the official policy, but they gave my H a refurbished phone for free to replace his phone when he cracked the screen. I <3 Apple. I think it really just depends on the employee.
I went to 3 stores with my cracked iPhone 4S. They all told me the same thing. $200 to swap it out. In the past I had luck flirting my way to a free replacement. Looks like it isn't going to work this time.

If you bought apple care, it might be covered. We both purchased Apple Care + & the CS rep said that anything DD could do to the phone would be covered for a $49 fee.

I was surprised, too. I might try one last time. If not, I think I will suck it up and keep the cracked one. I can upgrade in May.

Nope, because I typically think those are a waste of money. This is literally, in 10+ years of owning a cell phone, the first one I've ever broken or submerged or otherwise ruined.
Even still, it's still totally functional, just looks stupid, lol.
We pretty much only bought it because of DD. I figured $99 was well worth it for the amount of times she plays with our phones. Even if I never need it, I don't worry about her playing with the phones.

The Apple store replaced my 4 for free right after the 4S came out. The guy was originally going to charge me the normal $150 cost, but I think he appreciated that I told him honestly that I dropped the phone and it broke. I didn't try to make up a story.
FWIW, Consumer Reports says that AppleCare is worth it, unlike phone insurance in general. We bought it for our 5s.

This recently happened to me without insurance. They would not give me a new phone, they would only fix the screen for the same price they charge everybody. That was about $200. I went to a girlfriends husband for $50.
Also, I was told having someone else fix the glass voids the warranty. But that might still be worth it for you.
Yeah, I'm thinking about getting a non-iPhone but I have to wait until April when I can upgrade.
Check into how much it costs to buy out your contract. I have Sprint and it's $20 for every month left on your term. If your term is up in April, buying it out and getting the phone you want might be cheaper than fixing your iPhone screen (if batting your eyelashes doesn't work of course).
It used to be up to individual stores to decide what they wanted to do. Each store had to eat the cost of replacement phones. Apple now has a set price for replacing uninsured devices that have cracked screens. The individual stores have no input.
