We are leaving soon for a cruise. My mom is coming to stay with DS. I need to type up a list for her this week since I have some down time at work.
Here's what I've got so far:
-insurance card (new one starts 1/1 so we need to bring in new copies anywhere) -doctor's names, phone numbers, office locations -preferred hospital -what needs to go in the daily daycare bag -meals (rough idea-portions, sippy cup sizes, snacks, etc.) -ideas for play/places to go -how to do diaper laundry/diaper routine
You might call your dr's or ER to see if they'd need a note from you to treat DS or how that works. My sister was watching her nephew and had to take him to the ER for a fever induced seizure and the ER wouldn't treat him since she wasn't his mom. Not sure if that's normal or not?
List of any allergies or food/drinks he's not allowed to have? Not sure how old your DS is, but maybe favorite toys or ways to distract him if he's throwing a tantrum?
I agree with all routine info (morning, bedtime, etc)
Also agree with the "permission to treat" note
Since she is going to be at your house, maybe a "about our house" list of notes? Ie - programmable thermostat can be changed by doing xyz, backdoor lock sticks sometimes so you need to do x,y,z ... Just those funny little things that every house has, but that a guest might not figure out?
Yeah, at our pediatrician's office we have to sign a form for anyone else to bring him in for treatment and get medical information about him, I think. You might want to call to see if they have anything like that.
Also, our daycare requires a similar form for anyone else to pick him up.
You need to write and sign a note that you give her your consent to act as their guardian in case they need medical treatment. Include your contact info for the cruise. My mom had to wait for my sister to drive home 2 hours so the hospital would treat my niece.
A list of any meds she might have to give even occasionally and doses/frequency. You don't want her giving the wrong dose of Tylenol if she's used to different concentration. Make sure she is familiar with the carseat and how to tighten it Make sure daycare has her contact info and knows she is picking up No desitin in the CDs If she isn't local, a list or map of stores nearby
Thanks everyone... I will take care of the note as well. I remember growing up that my parents had it notarized for when they went OOT-do I need to go that far? It's not a big deal, there are two notaries in my office.

I'm pretty sure you do need to have it notarized.
We had a note that our doctors office provided, and we also wrote a second note for the hospital, and neither was notarized. And my parents, naturally, had to take ds to the doctor while we were gone.
Not to be morbid but we also made sure our parents knew where our will was.
