I emailed and asked if registry items without receipt could be looked up to know price paid for exchange.... Response is below.
Thank you again for your inquiry regarding our return policy.
Yes, by accessing your registry, the store can determine not only the price paid, but the actual store locations the items were purchased at, or if the purchases were made online. I hope this answers your question, and I definitely apologize for the confusion. Please don't hesitate to respond to this email if you have any further questions. You may definitely return or exchange gifts purchased from your registry.
Please read all of the following to make this process go more smoothly for you. If you are returning the gift to a store, simply bring your packing slip with you to serve as your receipt, and be sure to let the associate know that you were not the purchaser, so that you may get the credit, not the purchaser. If you prefer to return the item to us via shipping, we can process a prepaid return label for you that you may use to return the item or items to our online fulfillment center. Again, be sure to let the agent you speak with know that this will be a confidential gift return, in order that you and not the purchaser gets the credit.

Thank you, once again, for contacting the "R" Us Family.
I returned something to TRU yesterday without a receipt. Since my MIL bought it with her rewards card, I gave them her phone number and they looked it up and gave me a merchandise credit for the amount she paid (not the lowest sale price) since she paid for it with her Mastercard.
I was prepared to get screwed, but I think it depends on who is working. If you get a customer service person who is helpful, and they're not super busy, it seems to go more in your favor.
Just pray people that aren't considerate enough to include a receipt are on the ball enough to remove items from your registry, or you're back to screwed.
