Fresh is best. Take a ziploc baggie (double bag if you want) and turn it inside out and place it in your hand. After the dog does his/her business go and pick some of it up (you don't need A LOT of it!) then turn the bag right side out and close it up. Voila!
It's okay if it hits the ground. With all the fosters we've had I had to bring lots of samples in and I've brought samples in with grass clippings, sticks, etc. in the bag and it's fine.

Don't you usually pick up the poop anyway? Just bag and bring it in. And they only need the size of a tootsie roll. A vet I know gives people a baggie with a tootsie roll in it so people remember they don't need a huge sample. Kind of gross, but funny.
Lucy always Fear Poops at the vet, so I never have this problem. I just scoop it off the floor and hand them the paper towel. I'm sure they loooooove us
If she wasn't that accommodating, I would just take her for a walk and pick up whatever she was handing out that day.

No, we usually leave it hanging around forever
I didn't know how fresh it needed to be. We do pick up the poo but it's not something I'm doing daily.
