This is possibly flammable, and definitely a touchy subject so, uh, watch out if you're reading.
Does anyone know where we can find whatever forms you fill out in this state to designate someone as the person who makes medical decisions for you, and to spell out what to do if they are unavailable? Google gives me a bunch of semi-sketchy looking paralegal service providers (like legalzoom but sketchier). Maybe we get copies of these forms from the hospital where we're delivering?
Also is there some sort of equivalent of this for baby? Like, can we authorize that if msniq is not in a position to make a decision that certain extraordinary measures not be taken to keep babyniq alive if doctors determine that he will have an extremely short and unpleasant life? Note: NOT saying that this is what we are going to do, but by default it looks like they will take all available lifesaving measures.
The best option is to hire a lawyer and draw up a will and the accompanying paperwork that i can't think of the names of (find a lawyer that can remember the names of the paperwork .
h and i have been using rocketlawyer.com for our lease stuff for the house. it has quite a plethora of things, i know for sure it has a will.
we did a 7 day trial (enter cc number for it) need to cancel before the 7 days. the documents it spits out look pretty awesome. you can save it as a word doc.

You can also check with MsNiq's OB office to see if they have any fill-in-the-blanks, but it sounds like you have it covered.
ETA: Not flammable at all, especially IMO. After what we went through with M, I am going to make sure we have stuff in line that way just in case something like that happens again.
