I'd suggest crating him or keeping him with you at all times until you are 100% confident he won't have an accident.
How old is the puppy? After he relieves himself outside, are you feeding him again before he has an accident in the house?

Does he go upstairs with you? I'd put a bell upstairs and take the pup upstairs with you. Show him the bell. When he rings it, go right downstairs and outside.
Could he be ringing the bell downstairs and you just don't hear it?
