DH and I will be leaving on Monday for a cruise without LO.
Does anyone have a medical release form or generic doument they can share with me or tell me where they got it?
We want something saying that our parents are allowed to provide whatever medical care might be needed in case of an emergency and also if we are not able to care for our child or die (god for bid) they will be the caregivers. I know if we had a will, we would probably have something like this but we have not gotten around to that yet.

Whenever we have left the kids in the care of family, we have just always handwritten out a letter, giving our permission for any and all medical care to be adminstered in the event of an emergency. And with it all necessary information and signed by both of us. My mom is very particular about having this because she had friends who left their son with his grandparents, and he was injured and denied medical care because the hospital didn't have parental permission. But you have me realizing an actual form is what we probably really need, signed by a witness. If you google "Cargiver consent form for medical treatment" or something similar, there are several I just found. I'll definitely go that route next time.
