My leather cross-body strap black purse. I love that it just hangs on me and I don't have to worry about it whether rushing through the airport, on public transportation or just a day of shopping. Plus it's a classic in black leather and goes with everything.
Eye mask and my own pillow case from home. Having a familiar feeling piece of fabric under my head helps me sleep and it's so small it can fit in anywhere in my luggage.

Lotion, chapstick, emery board. I almost never have my nails done, but being stuck on a plane with a jagged, broken nail and no way to fix it is still a special kind of hell in my book.
If I am going somewhere humid, I bring sinus meds and a Vick's inhaler. But, I have chronic sinus issues.
I also like to bring snacks that pack easily in my purse- granola bars, peanutbutter crackers, dried fruit, etc.- since you never know when you're going to sit on the runway on a plane for hours before taking off or be in some other situation where you can't just buy food the minute you're hungry.
For our carry-on: hand sanitizer, travel pack of hand wipes, baggie or small bottle with (Aleve/Advil, Tylenol, Benadryl, Immodium), earphone dual adapter so DH and I can watch a movie together on my iPad, and the basics: granola bars, water that I buy before boarding, sometimes an extra long sleeved top or socks if I'm wearing flip flops or a tee.
Items that aren't standard and I always pack in luggage: nail polish remover, spare nail polish in the color I'm wearing, fingernail clippers, a reusable laundry bag for dirty laundry, box of granola bars, box of Clif Kids fruit sticks (dehydrated fruit), an empty tote bag/oversized purse for beach or if we need another carry-on for the return, face mask w/toiletries as I actually have time on vacation to pamper myself!

I always bring a small sampling of medication, even if I am not sick (i.e. cough drops, anti-histamines, cold medication, etc). I have gotten sick while abroad on two occasions and during the first I did not bring any medication with me. Searching for medication in another country in another language is very difficult and stressful, especially when you feel like crap!
I always bring a small sample size of aloe vera gel, just in case one of us gets sunburned.
I also bring several plastic bags to put dirty clothes into. On my Europe trips I am often city hopping so I have to constantly repack my suitcase and I want to make sure the clean clothes stay away from the dirty ones.
For trips that involve a lot of walking I make sure to bring moleskin in case of blisters, which has come in handy on several trips!
I also like to bring a product called After Bite which is used on bug bites to sooth the itching. It's very small and fits into my liquid bag.

Flashlight a lot of the places we visit have frequent power losses. Books, charger, drink flavor packets- drinking all water can get old, passports even traveling in the US.

Reusable water bottle. I hate paying for water, and I drink a lot of it everyday, especially when traveling.
Most of the stuff in this thread I'd never bring with me. I travel light. Also, I love buying drugs in other countries. They have the good stuff
Of the things mentioned, I do bring nail clippers, books (or kindle), sunscreen if I'm going somewhere beachy, pepto, chapstick. I forgot chapstick on our last trip, and we ended up buying it during a layover b/c we missed it so much.
In theory, a black camisole, fuzzy slipper socks, and nail clippers. The first two are things that I didn't pack when I went to Ireland and got stranded, and immediately bought to fill out my wardrobe to feel more "normal." I also tend to forget nail clippers and have to buy them at my destination. I've basically put those three things on my permanent list.
Ziploc bags. Extra pads/tampons. Band-aids. My immunisation record book. Extra passport photos. Copies of my son's recent medical records. copies of passports, residency permits. Flashlight. Travel umbrella.
It varies a lot by the destination! A few things that I haven't seen mentioned...
folding fan - great for hot destinations without AC, and very useful on planes, because they often get stuffy when they're stuck on the tarmac during boarding/delays
paperback book - I pack a huge library on my iPad Kindle app, but I make sure to pack at least one paperback to read on the plane during taxi/takeoff/landing or to leave on my towel if I'll be by the beach or pool.
blue/black pen - I always make sure to have a pen in my carry-on for international flights to fill out landing cards and customs forms.
plug adapters if necessary
rain jacket - I have a tiny umbrella in my purse, but I really like having a raincoat in bad weather, because it's easier to get around. I don't want to let the weather to spoil my travels.
compact reusable shopping bag - Whether it's purchases at the airport Duty Free or finding something unexpected when I'm out and about, I end up using this all the time, since it's easier to carry for long periods than most plastic bags. Sometimes I'll even take off a sweater or outer layer and put it in my shopping bag because it's easier than carrying it by hand.
travel speaker - If I'll be away for a long trip, I like being able to play music from my iPhone in my hotel room.
insect repellent - Some of the places with mosquito problems are surprising. I got in the habit of carrying a bottle of bug spray in my purse in Beijing, because the mosquitoes on the patios at night were killer. Off Family Care doesn't smell so woodsy, so it's a good option for light use.
shower cap - It's nice when hotels provide them, but since not all do, I take them with me when they're available and keep one in my travel toiletries in case I need it later.
condoms - Not the thing you won't to be wandering around a strange city to find. I just keep a few tucked into a suitcase pocket so I know I always have some.
