They look like this:
I've been using them to paint my bedroom, and no matter how many new ones I buy, the pad keeps falling off the handle! I've managed to manipulate it so that it works, but it sometimes still falls off. I'm only using them instead of rollers because it seems to be faster and I get a smoother finish.
Has anyone else experienced this or am I going insane?
I have only used the smaller kinds for edges. I think they are Shur-line (?) or something like that. Mine didn't come off though but those are smaller than the ones you are using. IDK. Maybe secure it by wrapping a piece of tape around the screw part and then attach the pad for a tighter fit?
I've tried the shur-line ones, but prefer the rollor (I guess I like the slightly textured effect the roller gives to the wall.) Sometimes the pads border on just smearing the paint on too and you can tell.

We do our edging with the Shur-line edgers, but I've never heard of ones that are for the entire wall. We've never had issues with the Shur-line pads falling off.
I use Shur-Line Handi Painter 1500C. THEY ROCK. I use rollers for the big parts but these things are awesome for edges and smaller areas.
My Lowe's stopped carrying them but HD has them. Look on the bottom shelf. They're about $2. I've found them to work a lot better than the regular Shur-Line replaceable pads or the thingy like you posted. The pad is attached to the styrofoam handle so no falling off.
I only use Shur Lin paint pads and have never had a problem with them sliding off if they are attached properly. I love the even coverage. I hate rollers. I've painted 3 houses using the paint pads and will never use rollers again. Make sure the pad is clicked-in over all the edges.
