I've been at my job for 3.5 months and today was the first time I used the fax machine.
I'm really embarrased that I had to ask someone to help me because it's not as simple as punching in a phone number and hitting send on the setup we have.
Never and I almost always fuck it up. I have also had to have student workers help me with the hole punch on more than one occasion. I tell them it is an example of how college pays off at the end. Some day they can hand that shit off to someone else, too.
Occasionally. We used to have fax machines all over my office and now we only have one, up front by the receptionist. I'm always amused at the people who ask me to fax stuff, still.

At work, we fax stuff at least once a week. It is the same with receiving faxes, we receive a fax at least once a week if there is no inventory. If there is an inventory, we get a couple reminders by fax.
I used to have to fax any FSA reimbursement claims, but once we got a new printer that will scan to either email or my personal file folder, I haven't faxed in months and doubt I will anytime soon!
Multiple times a day at work. We have recently gotten into the scan and e-mail thing, but many people we deal with do not have that technology or are just used to faxing.

Maybe once a year I fax something.
So a funny story about faxing. A teacher at my school (who is now retired) legit thought that when you sent a fax, there were tubes underground everywhere and the faxed paper would go through these tubes to its destination. She was so confused as to why the faxed paper was still in the fax machine! To this day, everyone tells this story.
Our office has been in a bit of an uproar since they cut the fax and it is now through a fax server app that only myself and my receptionist have access to. I hate it because if someone doesn't tell us they have a personal fax coming through, we open them and have access to way too much personal information. We actually get/send about 10 a day.
We used it all the time at the firm where I used to work. Of course, we also used Dictaphones, real stamps, and a huge 1970 IBM typewriter, so we may not have been the most technologically advanced law practice out there. (We did have normal computers and a copy machine, and 2 of our attorneys did know how to use email, just to clear that up).
Going Office Space on our fax machine was an ongoing dream of mine.
