Looks good. I would change the last sentence to be more like "I have attached the forms. I will contact you next week to see when I can pick them up. Thanks so much, bitch."

I agree with the contacting her in the coming week to schedule when you can come pick up the papers- this way she'll have less of a chance if pulling an "I don't know when I'll get to it."

Is there an HR person you could work with as well should she pull the bitch act?

I like what Thad said and change the last sentence. I would take out Could you, and replace it with "when you complete the forms.
