Where I live, there is a vet service that comes to your house for visits and such. I imagine the vet also would do euthanasia at home too. I'd ask my vet in advance if they do it outside of the office first. If not, I'd probably Google to see if there is a service like what I mentioned if that an option.

My parents vet came to the house for their cat. He got so so scared at the vet office and they didn't want that to be his final memory... Ask your vet. I'm sure they will do it.

I'm in Denver, so we're talking a large city, I have used a vet whose whole practice is doing home euthanasias. It was a wonderful experience, but not cheap (I think $300 or $400 for my cat, but that included taking care of the body). The whole experience took an hour, as she spent time with me and with my cat, allowed us to take our time saying goodbye. Even allowed my other cat to be nearby, though I know he didn't understand.
Our vet office does it by appointment and when the days didn't work for us we found someone else who came to our house. I forever dreaded taking our dog to the vet "one last time" so having the vet come to the house made the best of a horrible time.
