Gramercy Tavern is very, very good, but it's a 3 hour dinner, so not something you'll want to rush. If you do decide to go here make sure to book your reservation well in advance.
I'm not a huge sushi fan, so unfortunately I can't make recommendations there.
But since you like tapas/small plates, I want to recommend Danji as it's pretty close to the theaters. The negative is that they don't take reservations, but if you get there early, you shouldn't have a problem getting a table.
Another option for dinner, although not close to the theaters is NoMad. I recommend that because I just had dinner there for the second time and it's becoming one of my favorite places to eat. So good!
For hotel, I just got back from a trip where I stayed at City Club Hotel, near times square. I'd recommend the hotel to others.
It was actually a couple blocks east of times square, so it was quiet at night, but within 2 blocks of subway stations at Times Square, Bryant Park and Grand Central. That made it really easy to hop onto any line without having to deal with transfers.

If you like fish, Aquagrill which is right there is awesome.
DH and I went to the city for our anniversary in July and saw Book of Mormon! We stayed at "the Jewel" which I got from Priceline. We had dinner on Restaurant row and during the day just wandered around the city taking in the sights and a street fair.
Danji looks great, what time do you think we should get there if we have Book of Mormon at 8pm? And, is it just communal tables?
It's not all communal tables, but mostly communal tables. The restaurant itself is pretty small.
It's tough to say. I've gone where they said the wait is anywhere from 30min - 1.5 hours. If you want to play it safe, I'd probably get there close to the time they open.
One thing that is nice, is when you put in your name they take your cell phone number and will text you when your table is ready, so you don't have to be stuck waiting in the restaurant.
