I think my insurance covers a hospital grade pump but I need to buy it from an approved medical supplier and I will be reimbursed. I need to call to verify.
We got a Medela double breast pump, don't know the exact model. One of my best friends got it when she had her daughter 5 months ago and only used it twice, and my insurance doesn't cover one unless the baby is premature, so she is lending me hers. It's a $300 pump (her insurance covered it), so I'm happy with being able to use a pretty much brand new one for free.

I use the Medela Harmony as my manual pump, which has been really great (I was EPing for a few days until DD magically resumed taking the breast yesterday, but I'm still pumping for a stash). My double electric is the Medela Freestyle. Now that my supply is increasing, I'm finally liking the Freestyle - before, my output was better with the manual pump and I was getting milk out more quickly!
I don't know yet. Whatever my insurance covers. They recommended me waiting until after I give birth because I will have hit my OOP max, so will not be subject to a copay or anything.
Call your insurance company and see what they cover. They just needed a Rx from my doc.

Mine is a Medela double electric of some sort. It has been a huge hassle to figure out insurance coverage for it, but last night I think it finally went though. Should be arriving next week.

I EP'd for months using a Lanisoh double electric pump (Ameda Purely Yours knock-off). I planned to re-use it this time, but will be getting a new Ameda Purely Yours through insurance.
My BCBS plan covers $150 from one of three med supply companies. I requested catalogs, but what I'm hoping for is a Medela, either Pump in Style Advanced or Freestyle, leaning toward PISA. If I can't get that thru insurance, I may just buy or register for it.

I have a PISA. Two, actually...I happened to luck out and have a couple of friends unloading their gently used PISAs recently so I keep one at home and have one to keep at work. I have no complaints with it, it's great
