I had one done by an attorney when I was meeting her about probate court after DH passed. I didn't want my kids to have to go through what I was going through. I'm so glad I had it done. It's worth the expense.

i had to re-do mine, but my original ones were done my a military attorney when we were deploying, i just re-did them myself with no attorney help and had them signed and notarized.

DH and I both had them done through a lawyer. Besides the crap you could get into if parents or siblings disagree with you (a la Terry Schiavo), I think it would be easier to be able to say, "That's what he wanted," and live with a very difficult decision.
