My trainer actually had me do an exercise with the incline all the way up and holding on to the top, but I had to make the belt move myself - it wasn't on
Depends on why they're doing it. If I were training to do a mountain climb, then I might use an inclined treadmill to help. Holding on could help mimic the walking stick that I would likely have when climbing. If they are practically falling off the machine, then they are probably being a bit unsafe.
Your argument could be made for those who hold on to the elliptical or step machine too. While not the best way to use the machine, they're doing it, so more power to them!
There's a guy at my gym who has been there as long as I have, probably longer. He holds on for dear life on the mill when he goes fast. Every single time I see him it's the same thing. Now that is what I call dedication. Not everyone can use the mill day after day and year after year. It actually impresses me!

Are you talking about the people with steep inclines who lean back as they are walking? The only thing keeping them from falling backward are the handlebars they are holding? I see people doing this all the time at my gym. I have no idea what, if anything, it accomplishes.
