All of DH's friends are hooked on this show. So we watched the first two episodes last night. Perhaps it was because I was a bit on the tired side, but I got confused about the plot and wasn't sure after watching if I liked it or not.
There are a shitton of characters, so I agree it's difficult to follow. Honestly, I didn't really catch on until I read the books, then re-watched Season 1. Haha.
But I'd definitely stick with it (and read the books if you can)! The books are amazing in and of themselves, and HBO did a fantastic job with the adaptation (...especially the casting. OMG I <3 Peter Dinklage so much.).

Yeah, apparently they all read the books. That must be the key. (I'm sure the breasts on screen every five minutes doesn't hurt.)

I found the first few episodes hard to follow too. There are a lot of characters to keep up with. I have not read any of the books. I stuck with it though and it's one of my favorite shows. I found it helpful to go tothe HBO site for Game of Thrones and look at the character list with the photos of the actors. It helped me learn the names to go with the faces.
It's awesome! I also read up on the character bios online to understand it better. After a few more episodes, the main characters will really start to stand out.
We LOVE this show! Missy's right though, after a few more episodes the main characters really stand out. This is one of those shows though that really require 100% when watching it, if I try to fool around on the computer at the same time I spend the whole episode going "Wait... who's that? what's going on?"
It does take a few episodes to get the hang of just who everyone is.
I'm glad I watched the TV show before I read the book, which I'm working through right now. Having seen the show helps me put faces to the characters in the book, otherwise I think I'd really be lost.
It also took me a few episodes to get past all the sexual violence in this series. Sometimes I do divert my eyes because I'd just rather not see it.
I found it confusing at first too. But DH read the books and was able to help me some.
But past that, what I did - first, I went on-line and read up about the characters and tried to read back-ground information. Seeing the names in print helped me a TON in remembering who was who, etc. Their names are just odd enough it was hard to make sense w/o seeing them in print.
Then I discovered watching it on my iPad through HBO Go. There is this whole interactive feature that you could have on the screen at the same time and there was a chart of the different houses, etc. Having that in front of me as they were rattling off names and talking about this house and that house - also helped a TON in figuring out who is who and who they are related to and who they hate, etc.
