Under my shirt for the sole reason of the fact that I am not slender and I feel that my fluffy belly protrudes too much if I have it around my waist over my shirt. Vain? Yes At least if it is under my shirt my belly isn't being cinched in tight because my shirt can flow over the belt.

I wear my SpiBelt during any race or training run over 10mi - basically, any time I will need more than one gel. I typically do not run with my phone. I mostly use it for gels, but also keep those cottenelle single wipes in there. If my DP aka race sherpa is not with me, I will also use it for my car key, hotel key, etc. If it involves a bus ride and/or long wait before start time, I'll throw my inhaler in there too.
It almost always goes over my shirt. If it doesn't have much and is laying flat, I might put it under.
I'm going to have to figure out a fuel belt for marathon training come June.
I have a spi-belt. I only wear it for races over 10k. I stash: tissues, tape (if my foot's bothering me), sport beans or Sour Patch Kids (don't hate), cash/credit card. I wear it over my shirt.
I wear a spi belt for my phone for all of my runs. I wear it low on my hips, so it's over my capris/shorts/skirt but usually under my shirt. If I wear a fuel belt, I wear it over my shirt at my natural waist.
I have a waist belt, I wear it for anything greater than 8 miles, which is when I need to carry gels. I wear my phone on my arm as I use it for Strava and music. Waist belt is worn around my waist - it's most comfortable there. Also, I always wear it OVER my shirt, never under. It would chafe and I need o access while running.
