I registered for the basics, like a pump, a Boppy, bottles, and storage bags.
Nursing tanks, pads, and nipple cream felt a little too personal for me. I figure I'll just use gift cards for that. I think my CWs are going to do something and I'd feel a little weird about them buying me nipple cream, ha.
I think pads and creme are too personal, but if you are okay with it that is fine. I have seen it on people's registries before and only thought that she was more okay with it than I was.
If you wouldn't think twice about registering for formula feeding stuff (bottles, warmer, dispensers, etc), don't think twice about breastfeeding stuff. It's part of the long, long cultural road to making breastfeeding normal.

Just a heads up I have XL Target nursing tanks and the bra portion is too small. Splurge & get Bravado. If you know what size you are you can order from Target.com.

I think it is totally fine. My aunt, who is really modest and easily embarrassed, bought my breast pump and nipple cream. She got a kick out of buying me something that I wouldn't expect from her.

As long as you are okay with the visual image of you actually using the nipple cream and pads etc. that will invariably appear in the mind of each person who reads the registry and/or sees you opening such gifts, then go for it. To me, it feels very personal and it also feels like it shifts the shower from being about the baby to being about you. Of course we all know that you need these things because you're having a baby, but really, who wants to buy nipple cream and bring it to the shower? In my circle of friends, I have seen things like pumps/pads on registries but never clothes for the mom - nursing bras/tanks etc. (As you may have guessed, these are never the items that I choose to purchase )
I really like the idea of adding those items after your shower so that you can get the discount. But really, I think people pretty much just do whatever they want when it comes to registries.

Yep, totally fine. One of my guy friends and his wife bought me a save the Tatas gift. Gave me pump supplies, nipple cream, etc. made me laugh and I use it all!

Completely okay in my world. My mom actually questioned me registering for a pump and I chastised her for it. She then felt silly when some of my good friends pitched in and bought it for me. That gift was used multiple times a day for 9 months, so god knows they got their money's worth!
Totally fine. I didn't particularly care about the visual people got while looking at my registry. And after, I think whatever they imagined was not nearly as embarrassing as the time a male, 25-year-old coworker commented on the slurping sound my breast pump made (he sat right outside the office where I pumped).

It's totally fine. I registered for the pump, extra horns, pads, cream, nursing tanks, all of it. Last year I made a BFing supply basket for DH's coworker's wife (including little extras not on the registry) and no one was weirded out by it.
Yes, totally fine. I registered for a Boppy and BrestFriend, got them both, and have never used them!
IMHO, I wouldn't register for a pump unless you're very sure of the kind you want/how BFing will go. I rented a hospital grade pump for the first month while I figured out what kind of pump I wanted, and I'm glad I did. I SAH, and I thought I would be pumping a bit more so I could get out of the house more. Turns out I really like BFing and it's so easy to feed him on the go that I rarely pump. I have the Medela manual pump and I really really like it. An electric would be total overkill for me. Plus pumping SUCKS.
You've gotten lots of positive response, but I wanted to add another vote for putting it on there. It's not too personal, it's a fact of life with a baby, that's one of 2 ways for them to eat.
