This is my first time making a clicky poll over here so I hope it works.
Let's say you and someone else are walking toward the door at Dunkin Donuts from opposite directions. He gets there about five seconds before you and opens the door for you (obviously motioning for you to go ahead). You say thank you and walk in ahead. Do you get in line ahead of him? Or offer for him to go in line first? If it matters, you have a simple order and the line is short, so you won't hold him up by very much by going first.

I've occasionally had this happen, and whenever I've offered to let them go ahead of me, they decline. So I'd probably just go right ahead and get in line, unless the guy was elderly. Then I'd at least offer.
Our DD has an inner door so I hold that door for the person to return the favor. This happened when we went to Longhorns recently, but when we tried to let them go in front of us through the inner door they insisted we still go ahead. So we did but I offer to let people go ahead.
I usually offer for them to go first. Mostly because I love it when people are nice and do things like opening doors, so I try to repay the favor in order to encourage them to keep doing it. They usually decline, but I still think it is nice to offer.
