How small is the dog? I ask because we've had two different solutions to this problem. We have a 17-year-old senior cat, a 20-pound fatty, and a 30-pound corgi-mix (long and low) dog.
#1 - we cut a small hole in the door. Trimmed it out, painted it ... voila - cat hole! The trick was making the hole big enough for the fatty to squeeze through but NOT big enough for the dog to fit. #2 - our current solution is a baby gate placed about 6 inches off the floor. Both cats can squeeze under, but the dog can only get her head under.

Hmmm. I'd try the baby-gate thing first, then.
In order to let the fatty in and keep the dog out, you could try doing a custom hole in the door. Measure the maximum circumferences of each critter and then figure out what size hole would let fatty in and keep the poop-eater out?
