Hi my iPhone locked me out last week and restored itself and I lost everything

I'm auto logged in on my iPad, but on my phone I'm not logged in.
I made the password in May 2012 when this started so I have no clue what it is and can't seem to find a way to reset so I can log in from phone.

Thanks in advance for any and all help.
