If there is a safe, I put it in there. If not, I zip it into the lining of my suitcase and hope for the best. If I'm in a place where that seems like a bad idea (um, usually not Europe), I throw caution to the wind and carry it with me. Yes, I know it can get stolen, and yes, that would be inconvenient, but it really doubt that it would be the end of the world, in fact, it would probably end up as one of those crazy stories that gets retold for the rest of my life.
I carry mine with me. Exactly how depends on the country (sometimes my regular bag if it is pretty safe, more often in an under-the-clothes waist belt.

In my money belt, under my clothes. Unless I'm going to be swimming or something. One time I did leave it in the safe for the day, and that one day ended up wanting to stay out and spend the night in another city... but you can't check into a hotel without your passport in Europe, so we had to cut things short. Not a mistake I intend to make again.
I keep it on me. I have this awesome indestructible Timbuk2 messenger bag that feels very secure. There's an inside pocket where I keep my passport and money. We've stayed at apartments and places with no safe, so I just got used to keeping it on me.
