Cheese and eggs. Nuts in anything. Carrots mixed in with other vegetables (fine by themselves). Peas mixed with anything (fine by themselves). Ricotta cheese and spinach.
I can't really think of food combos that gross me out. I think Miracle Whip should be outlawed though, it can make any food disgusting. I also dislike when foods aren't paired how I think they should be. I think certain main dishes should be served with certain side dishes and it drives me insane when we go somewhere and the main dishes and sides don't pair how I think the should. I blame this on my mother, who would serve things like cheeseburers with a side of peas. It just doesn't go together to me.
fish and tarter sauce (love fish, hate tarter sauce) pizza and ranch
raisins and peanut butter dont gross me out per say, but i do not want squishyness in my peanut butter, blech.
haha, when i was kid, my brother's favorite food was bread and ketchup. he would take a piece of texas toast and smother it in ketchup. it was so gross! he also loved to eat silverdollar pancakes. he would literally put them in a bowl full of maple syrup...it wasnt until a few years ago that i could tolerate the smell of syrup.
I don't like sugary cereals or putting extra sugar on plain cereal.
MH never heats anything up if he takes it out of the fridge. Cold pizza or chicken is one thing, but a few days ago he was eating cold leftover chicken and dumpling soup. BARF.
My h eats chili and peanut butter sandwiches. I think that's odd. Other than that, I eat anything.
This is funny - we had a chili cook off at work at my first "real" job, and everyone was like, "Who'll bring the PB sandwiches?" I was so confused, and they acted like I was crazy for not knowing that they "go together". What the what? It must be regional or something. I was new to the area, and no longer live there.
I don't understand buffalo chicken and ranch dressing. Why would you ever pick ranch when bleu cheese dressing is available and clearly the better option?
ETClarify: I don't really like ranch dressing on anything, so the thought of ruining buffalo chicken with a substandard dressing would gross me out.

When I was in elementary, we would have a half PBJ sandwich on days when we had chili. Not sure if that's really a "thing" but that's how it was in my school. maybe people dip their sandwich in the chili? I guess spicy plus peanuts isn't that odd, such as in thai cooking.
Peanut Butter & Cheese (my husband eats this kind of sandwich and feeds it to our kids - blech) Ketchup & Eggs Mayo & Vinegar (like when people put vinegar on their subway sandwich after they've already put mayo on it)
