I have two stories from when I was PG (there's my excuse) and after that I stopped being nice and packing for DH.
1) I was about 5 months along and we had a business dinner with stay at the beach for DH's company. He asked me to pack his suit. We got to the hotel room 1 hour away from where we leave and as we are getting dressed he realizes I packed the wrong suit. This one didn't fit. He was this close to cancel the whole thing but felt bad and rigged the pants with the belt and we had a short night. What a good sport.
2)We were going to a wedding 4 hours away and Sara let me borrow a beautiful maternity dress. I was about 7 months along. The dress and suit were in the hangers from the dry cleaners. We had some kind of argument when we were leaving. About a half an hour before getting there and with just the right amount of time to check in and get ready. My DH looks in his rear view mirror and asks me "where are the outfits?". Well during our argument we both went back and did a sweep of things to pack but one of us (we don't know whom) had hung the outfits inside the closet so we didn't realize they weren't packed.
At this point we just drove directly to a Close by mall and were lucky enough to find a suit for him that needed no alterations and a nice top for me. I also had to pay a salon for a quick Up do as there were no outlets in the restroom and no apparent way to do my hair in a hurry. We made it 15 minutes late.
DH and I both forgot to pack him boxers on his last business trip. He had no car and was in trainings and seminars from sun up to sun down so he went commando all week. I felt bad but it was actually pretty funny.
We are always forgetting something but usually just stop and pick it up when we get wherever we are going.

The worst for me was one time we drove the 8ish hrs to visit H's family, which we do a couple of times a year, and forgot our clothes bags. We had our bathroom items, which I always pack in a separate backpack, and we had the kids clothes and PNP. But no clothes for us. When we arrived at midnight, we unloaded the kids and ran out to get a few items.

when i was in 6th grade, we went to disneyworld for spring break. i packed my bag and left it on the floor in my room for my dad to put in the car. when we got to the airport, we go to check in and my dad asks where my bag is. he left it on the floor in my room and he had to drive back across town in a big hurry to make it back and get my stuff.
My husband was supposed to fly in and out of New York last month for a meeting - it was going to be a six hour turnaround. He packed nothing. He didn't even bring a laptop or phone charger. A crazy storm came through and the airline canceled every flight to our city that night and into the next day. He was stuck in the city for two days with pretty much just his driver's license, iPad and good looks.
