I used the Crest white strips (professional effects) then I tried the CVS ones...I really like them! They dissolve in 5-7 min. (I used in the shower) and weren't harsh like the Crest ones...I used them for the wedding I was in, and saw a huge difference.
I also gave up my (very bad) diet pepsi habit. I drank at least 6 a day! I've noticed a difference in my teeth (and skin, and nails, and other things)
I got a bleach kit through my dentist. I did it for two weeks as instructed and the results were phenomenal. They said my teeth were so white, they were off the demo teeth. I think it was called Night White.
Here's a link to the refills but I'd see if you could find a dentist that uses this system to ensure quality control. Also, you need to have molds made to have the bleach trays made.
ETA: Also needed to add that the dentist told me to wear them for 2.5 hours but I was able to go about my business around the house. I think I even wore them to the gym to do a light eliptical workout. I will say that my teeth hurt like crazy afterwards but looking back and reading the instructions, I was only supposed to wear them for 30 minutes. Not sure where the dentist got 2.5 hours from
Also, my results lasted forever. I didn't really use any products afterwards to keep them white. I think they stayed white for several years until my coffee consumption increased. For cold drinks I just drank through a straw to prevent new staining.

Whitening trays from my dentist and I maintain using Colgate Optic White. It contains a 1% strength version of the whitening agent I get from the dentist (which is 16%).
I use Crest Whitestrips on an as needed basis. I can't do a full treatment (makes my teeth too sensitive). I did a full treatment years ago and it made a very noticeable difference in the whiteness of my teeth. Since then I just use the strips for a few days a couple times a year. I currently use Crest Vivid toothpaste.

I tried the store brand of the Crest White Strips and they didn’t do jack…other than make my teeth hurt. Then I switched to the Rembrandt toothpaste, mouthwash & gel trays. They are very uncomfortable & you have to sit with them for like 20-30 minutes. I was wadding up tissue in my mouth to stop the saliva from coming out. Lovely, I know.
But they worked! And my teeth weren’t nearly as sensitive as they were from the strips. Just make sure to rinse with a fluoride mouthwash to prevent sensitivity.
I swear by Crest White stripes, the 2 wks kit; like pp's they do make my teeth sensitive so I either only use them one week at a time or use Sensidyne for my toothpaste.
I've tried the Target brand equivalent and they didn't do a thing
I use the optic white toothpaste, I think I'll start using the crest strips or Rembrandt gels. My dentist is the most updated in whitening options. Thanks ladies.
I wanted to add that I have used the Crest Whitestrips as well, and if you have significant teeth sensitivity you might find it works better to use the really low % strips instead of using the higher. I had much better results using the low% strips for 3-4 weeks (yes, that is a couple boxes) over the higher % that the dentist has every other day.
The sensitivity is why I like the mouthwash. I am prone to sensitive teeth, but the mouthwash doesn't affect them. Plus, I went to a family reunion a few weeks ago and had three different people ask me what I do to make my teeth so white.
I use Smile Bright which I actually bought through Groupon. Works great. I do it when I feel necessary because out of all the whitening systems I've used in the past, I've found this one burns my gums worse than any other. If you have sensitve teeth and you have a whitening kit with trays, you can put sensodine toothpaste in the trays for 30 mins prior to doing the whitening and that helps a lot. On a day to day basis, I also brush/garge with hydrogen peroxide. I have a friend who has never used a whitening kit in her life but uses peroxide daily and she has the whitest teeth I've ever seen!!!
