This has happened many times to us over the years. Once the diaper area is completely dry I put on triple cream paste and then I layer (not really rub) aquaphor over that.
When things are super bad and painful as you describe, I changed the diaper in the shower and then disinfected after. You want to make sure everything is washed off so the yeast have nothing to feed on.

I second a yeast cream and a lot of heavy duty diaper cream (not sparingly). Our pedi recommended Pinxav (there is a locator on their website www.pinxav.com/). We used several different OTC yeast creams in combo with the diaper rash cream, starting with Monistat. If you don't have a room without carpet, can you spread out a sacrificial blanket or towels and have her play on there with you for a bit naked? GL!
I'm sorry your kiddo is hurting so much. How much milk is she drinking daily? My DD was still drinking a lot of milk when she was two. We cut back on her milk intake and she significantly increased the amount of solids she was eating. It may be something to try if you think milk/lack of solids is contributing to the runny poop.
Do you see an improvement when you use the diaper cream from the doc? Could she be having a reaction to the cream?
Poor baby. The scream from a diaper rash butt is so awful. I can pinpoint it from across the house with my kids. I may not know they have a rash, but when H goes to change a diaper, I know it immediately. I hope she gets some relief soon!
If you don't have a room without carpet, you could put down an outdoor tablecloth or two in one room and stay in there for awhile, too.

Yes! I'm sorry for your kid's butt. It is the worst feeling. We did the all purpose nipple (?) cream: equal parts lotramine, polysporin and hydrocortisone. Saw improvements in the open sores within the day and cleared up in a week of consistent use, after THREE MONTHS of doctors jerking us around.

Also, we had been prescribed two topics antibiotics, oral antibiotics and a topical steroid (or two?). And none worked. I would try the combo above before another/more prescription. It seems to be a cure all.
If it's not yeast, try Sensicare. DS had a terrible rash when he was a newborn (which basically meant he was leaking poop all day) with big red weepy sores, and this was the only thing that stuck to it.
Put baby powder on top of the rash cream. It helps hold the cream on (particularly on those weepy parts where the cream rubs off easily) even when the poop so the poop doesn't touch the skin. The better the cream stays on, the more likely the rash will heal.

DD2 had an allergic reaction the first time we tried disposable diapers (we CD but our washer broke). It was awful with opening weeping sores within 24 hours. We used the grovia Magic stick and lots of semi-naked bum time. You could try gerber training pants (or similar) to help contain mess but allow for airflow. A little naked time in the sun helped too.

After you put on the diaper cream put cornstarch on over it. My mom learned this trick from my great grandmother and it works wonder for us. The worse the diaper rash it, the thicker I cake on the diaper cream and cornstarch. I also will put my son in the tub when he needs a diaper change so that I am not rubbing to get the poop/cream off. After 10 minutes in the tub I will gently wipe a soft washcloth over the area and everything comes off. Do you have any of the plastic things you put down when you paint? In a pinch I have been known to put one of those down with some towels over it (preferably immediately after a BM) so that my son can get as much naked time as possible.

Seconding trying Sensicare Barrier Cream - we've had amazing luck with it and were introduced to it in the NICU. It is the only barrier we've found that is still on the skin when we get to the next diaper change. You can get at some pharmacies or DMEs, but definitely through Amazon.

We just got rid of a yeast rash by drying off her butt completely, putting on clomitrizole cream (anti fungal) followed by Desitin at every change. I'm sorry, that sounds just awful, poor baby.
