This is what the lady at Rite Aid just asked me when I was checking out. I have Colin in his wrap, but you can clearly see his face. I was buying gas drops and saline nasal spray for him.
What? No, lady, I'm carrying around a doll and buying medicine for it.

Oh, don't you worry, I'm SURE there are people out there that do this.
There are. They will also go to the store and buy medicine for them if they have the sniffles, say they're fussy and need a nap, etc. The world of reborns is, um, fascinatingly frightening.
We went to this hippy dippy co-op store and the owner was walking around in a similar wrap. Except she was wearing the saddest little poodle I've ever seen. She kept trying to talk to N, who was not having it. I was all, "oh she's scared of dogs", which was a total lie. I didn't want to make the lady feel bad, but it was a pretty weird scene.
People asked me this allll the time when my dd was a baby. She was also tiny so they also asked if I was sure she was as old as I said. I dont get people.
I got that question when I had ilya in the wrap one time. His feet were dangling out and this woman asked if that was a real baby in there. Lol. Uh, yes. No, I just like to wear fake baby legs dangling from this bulge on my front.

I got asked more than once if DS was real while wearing him in a wrap.
On a creepy, realistic doll note... Sunday at work, one of the tables had a little girl, maybe 4, who I SWEAR had one of these things. She would stand up and put in on her hip and its head moved around like a real baby's would, it looked so freakishly real in the face and everything.

On the topic of baby wearing and poodles actually:
I've worked in the Arctic where babies are worn in the back of their parent's jackets called an Amauti. On one of my trips I noticed that a pile of the little kids were wearing the Amauti jackets as well and that there was something in there. They all had little Shih Tsu and toy poodles on their backs! When I asked why they would have itty bitty dogs like that in the Arctic a local explained that the kids carry them around to keep themselves (the kids) warm. Some of the kids also had dolls so they could be like their Moms. It was adorable.
Gratuitous photo of Amauti in action. - This is what most of the modern ones look like although they add an awesome fur collar during the winter.

I wore my daughter and some people had the weirdest response. One woman pointed at my colorful woven wrap and said "I like this. It's very...ethnic." with a lowered voice and a sneer on the last word. "Yep! It's German!" I said instead of rolling my eyes.
Someone asked if E and J were twins the other day.
Okay, granted, we were in the car, she hadn't gotten a good look at E, and I had mentioned they both just had birthdays. But it still made me lol. Why would you assume they were twins just because they both had birthdays last month? There are only 12 months in the year! Use your eyes!
Someone at the grocery store asked if my sister and I were twins. I was in college, she was in middle school. We're 5 years apart, she's 7" taller than me. Yeah, no.
When DD was six months old I was at a little shopping area in another part of town. I was wearing her, but she was awake and watching everything and everyone as she does. In different stores people startled when they saw her move and said, "oh! It's a real baby!" The last one I was in a baby boutique and the surprised person was the owner. You're surprised a brought a real baby into your baby store? Lol. Maybe it's common in that part of town to carry around dolls, I don't know.
People are weird. Or just really accepting of the Reborn doll movement. I got sucked into YouTube videos of those people once. Fascinating. They would totally buy their dolls gas drops.
