Agree that you don't need a big gift since you did the pictures. I would do A&A bibs and diapers. Or maybe a basket of infant Tylenol, gas drops, diaper cream, training toothpaste, wash cloths.
I like giving Trumpette socks. They're super cute, actually stay on feet, and kind of special and not something many of my first time mom friends are aware of.
We got a ton of bath stuff, which I loved. SIL gave us a basket of the essentials and I am forever grateful. I don't think we will need to get those items for a while.
Are they planning to breastfeed? I'd go with breastfeeding supplies. Lansinoh makes a starter kit that my sister got me that was awesome. People don't usually buy that stuff off registries but it is so needed right away!
Anode good thing is an activity mat or bouncer. We use those every day still! My baby loves the Oggi ball and the Manhattan toys winkel chewing toys.
I stay away from blankets and clothes. We got way too much of that. Especially blankets, we have a bunch we still haven't used and don't need.

Do baby boys get yeast rashes? I can throw in a tube of the generic lotramin, too.
They can--my first had yeast rashes a few times. My second didn't but probably because I was obsessive after the first
