So we are convinced D is allergic to tomatoes. The only thing that happens is that his face turns read and splotchy. Is this something that could get worse with time?
We are planning on taking him to an allergies in January, but until then, just avoid tomatoes?

This. If it's just a splotchy red area, it's probably just from the acid in the tomatoes, not an actual allergy. DS gets red splotches from anything acidic. Humus, tomato sauce, sauerkraut, pickles. I don't restrict these foods b/c it's not a actual allergy.
DD's pedi said that her red rashes after tomatoes were probably because of the acid in them and that tomato allergies are extremely rare. She's been tested for allergies to some of the common allergy foods and is allergic to peanuts and egg whites and the reactions to those have been much more extreme (vomiting, red, eyes and swelling). We still feed her tomatoes (her favorite food) and she does not usually get the rash, but I'd maybe snap a picture of the reaction and talk to her pedi or the show it to the allergist if I were you.

So this must be what's happening, because it did happen with BBQ sauce, salsa, pasta sauce, etc.
We really noticed it when we gave him ketchup for the first time, and since then it's gotten worse. We tried to give it to him one more time, followed by salsa the next night, hence the reason we were convinced it's tomatoes.

DS has too much histamine (produces too much?) in his system so he has contact reactions with some foods. He even tested positive for some allergens with prick testing, only to find with blood work that he's actually not allergic. Cinnamon still makes his mouth and face red. (Cinnamon sugar on his toast.) That's what this sounds like. I think tomato allergies are pretty rare? For a while blueberries made DS's face react, and the allergist said there's nothing in blueberries that humans would be allergic to.
DD had the same reaction for awhile. Same with ranch dressing, ketchup, and anything else too acidic. She grew out of it and it hasn't been a problem since!

What causes it?
As I said, it's a skin irritation caused by the acid in the food. Some people just have more sensitive skin than others.
That happens to me, and I haven't grown out of it, but it t is a mild annoyance and embarrassment rather than anything else. I've been tested for allergies and those tests have come back negative. I'd talk to his doctor about it, but it doesn't sound like anything that is going to seriously hurt him.
DD1 had this...clementines too, as well as eggs and green beans. So strange. She does have eczema. Took her to the allergist bc the pedi was concerned about the eggs and getting a flu shot but didn't test positive for anything.
